Making an appointment at California Imaging & Diagnostics is simple and easy. Once your doctor has provided a referral, scheduling your appointment at California Imaging & Diagnostics is simple and easy. You will need a referral form or requisition form from your doctor, and you may need pre-authorization from your insurance company or HMO.

Once your referral is in place, your doctor’s office can call to schedule on your behalf — or you are welcome to call us to coordinate scheduling after your referral has been issued. Our scheduling department is available from 8:00am – 5:00pm and can be reached toll free at 866-419-3372.
