Before Your Appointment

All visits to California Imaging and Diagnostics require a referral from your physician. Once your doctor has referred you to our clinic, contact your insurance company to determine whether a pre-authorization is also required. We will work directly with your referring physician to help obtain any necessary authorizations.

1. Insurance Pre-Authorization or Referral:

  • Referral Required: A physician’s referral is required for all services at California Imaging and Diagnostics. Once your doctor has referred you to our clinic, check with your insurance provider to confirm whether a separate pre-authorization is also needed.
  • Contact Your Insurer: Contact your insurance provider to initiate the pre-authorization process. We are here to assist your referring physician in coordinating with your insurer to obtain the necessary approvals.

2. Gather Required Documentation:

  • Bring the following items to ensure a smooth registration process:
    • Insurance card
    • Name and address of your insurance provider
    • Name and address of your claims payer (if different from your insurer)
    • Your ID and group numbers
    • Name of the insurance policyholder or subscriber
  • For workers’ compensation claims, please bring your approved C-9 form.
  • Important: All examinations require a written order from your referring healthcare provider.

Day of Your Appointment

Arrival and Check-in:

  • Timing: Arrive at least 15 minutes before your scheduled appointment time.
  • Preparation: Follow specific instructions for your examination, such as fasting or withholding medications.

What to Wear:

  • Wear loose, comfortable clothing without metal fasteners, which may interfere with imaging equipment.
