Purpose

This document provides the inline consent language required under the Telephone Consumer Protection Act (TCPA), 47 U.S.C. § 227, and FCC implementing regulations at 47 C.F.R. § 64.1200. This text is not a standalone webpage — it must be inserted directly into any website form that collects a phone number, immediately above the submit button, in a minimum font size of 11px.

  • Do not reduce font size below 11px
  • The Privacy Policy link must be a live hyperlink to cidrad.com/privacy-policy/
  • Retain a dated record of the exact disclosure text in use at each form location — this record is important in the event of a TCPA compliance dispute
  • If CID sends automated SMS appointment reminders, confirm that the scheduling platform (OpenDR) logs consent at the time of collection and maintains records of patient opt-outs
