Schedule An AppointmentMaking an appointment at California Imaging & Diagnostics is simple and easy. You or your doctor can schedule your appointment. Either way, you will need a referral form or requisition form from your doctor, and you may need pre authorization from your insurance company or HMO.

InsuranceCalifornia Imaging & Diagnostics is an approved provider for most major insurance carriers and health plans. We will submit your claim, and any secondary claims, to any insurance company under the benefit coverage you specify, including workers' compensation, auto insurance, and personal injury. Ultimately, the patient is responsible for payment of the account, including any balance not covered by insurance. We will send you a statement listing payment made by your insurance company as well as the balance you may owe. Please contact your insurance company if they have not paid for your procedure after 30 days.
To streamline the registration process and minimize your personal expenses, it is important to take the following steps before your appointment.
Step 1: Find out if your policy or plan requires a preauthorization or referral. If so, contact your insurance company and find out what you need to do to obtain one. We will assist the referring physician with the process of obtaining the authorization for your exam. Step 2: Come to your appointment with your insurance card, name and address of your plan or insurer(s), name and address of your claims payer (if different from insurer), your ID and group numbers, and the name of the policyholder or subscriber. If applicable, for workers' compensation, your approved C-9 is required.

Why do I need to arrive at least 15 minutes before my exam time? It's necessary for you to arrive early in order to complete your paperwork and prepare for your exam.
What type of clothing should I wear for my exam? Wear comfortable, metal-free clothing (i.e., sweat pants, T-shirt, shorts, metal-free bra, etc.). It may be necessary to have you change into a patient gown or scrubs depending on the area being scanned. Secure lockers will be provided for your clothing and valuables.
SCHEDULING - How soon can I book an appointment with California Imaging & Diagnostics? Can I fit my appointment into my work schedule? Our centers are open from 8:00am to 5:00 pm. In many cases, we can schedule you for the same day you call for an appointment. Our goal is to schedule your appointment at the best possible time for you.
PAYMENT & BILLING - What do I need to bring with me to facilitate the payment process? Bring your complete insurance information, including your insurance card, ID number, and group number. In addition, for workers' compensations claims (C9), bring your claim number, and the date of injury.
Do I need preauthorization from my health care policy or plan? Some plans do require preauthorization. Check with yours to see if this is necessary. Your insurance company can also answer any questions you may have about what is covered and what is not.
I don't have medical insurance. Can I get a price quotation on my procedure? If you do not have medical insurance and/or you are concerned about paying for your procedure, call our toll free number at 866-419-3372. We'll give you an estimate and work with you in setting up a payment plan.
Does CID file secondary claims as well as primary ones? Yes, we do.
I have a question about my statement who do I contact for information? Call the Revenue Management department with any of your questions regarding your statement at 1-888-747-2228.
MEDICAL RECORDS – How does my physician get the results of my exam? CID is completely digital. We provide reports and images to your physician via a secure website. We also fax a report to your physician immediately after the exam has been read by our radiologist.
May I get a copy of my films? Yes. We provide one complimentary CD of your images. There is a nominal charge for additional CDs. Printed reports are also provided upon request.
How long does CID keep my records on file? We maintain digital copy your images and reports from your exam(s) in accordance with state laws (in most cases, for a minimum of five to seven years).
PrivacyPatient Privacy - HIPAA At California Imaging and Diagnostics, patient privacy is taken very seriously. We comply with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy Rule. The Privacy Rule provides federal protections for personal health information held by covered entities and gives patients various rights in regards to that information.
